When appropriate for the resident’s needs, outside home health or hospice services can work alongside the daily support already provided in the cottage.
With consent, we help maintain communication among the resident, family, Happy Sunflower caregivers, and the family’s chosen provider. Eligibility and clinical decisions remain with the licensed outside provider.

No. Happy Sunflower is not a home health agency or hospice provider. We provide residential daily care while your family chooses the licensed outside provider, and we coordinate around that provider’s plan.
What can Home Health&Hospice Coordination include?
With consent, coordination may include sharing practical updates, supporting visit schedules, maintaining daily routines, and helping the family and outside provider stay connected.
Can coordination help a resident remain in familiar surroundings?
It may, when the residential setting remains appropriate for the resident’s needs and the outside provider’s plan. Suitability and availability must be confirmed for each person.
